Office Policies

About Your Insurance

The office participates in most insurance plans, including Medicare and Worker's Compensation, and will help you get through the paperwork your plan requires. We will ask you for your insurance information when you call for our appointment. This allows us to contact the plan so that you can understand your responsibilities, such as co-pays and deductible. Please note that these co-pays and deductibles are set by the insurer and not by our office. Also please make arrangements to pay your co-pay at the time of your appointment to save you the added costs of billing..

At the time of your appointment please bring

  • Insurance information and your insurance card
  • A picture ID
  • Any previous tests (You can have it faxed to 401-946-7230)
  • A list of your medications

In the Patient Forms section of our website you can fill out your medical history and mail or fax it to us. In addition to saving you time at your appointment, we can see if there are any tests or other information that we need to obtain.

We ask you to confine after hours calls to true medical emergencies. No prescriptions or prescription refills are written outside of our normal business hours. All prescriptions will go to the one pharmacy the patient identifies.

There is a $25 cancellation fee applied for appointments not cancelled within 24 hours of the scheduled appointments. This is to ensure that if you are unable to make your appointment the time can be used for another patient. 
 

Policies

We encourage you to learn about your policy, to prevent surprises and let you prepare for any additional costs that you might expect. We will do our utmost to help you, and the more you understand the easier it is for everyone involved. Please remember that these policies the result of insurance company rulings.

Referrals

Some policies require you to have a primary care physician's referral before you can be seen. Please contact your PCP so that we can receive this in advance of your visit. If we do not receive the referral, you visit may have to be postponed.
 

Worker's Compensation

If you think you have a work-related injury, you must fill out an Injury Report through your workplace. They will arrange for it to go to the appropriate Worker's Compensation insurance company, who can authorize treatment.
 

Deductibles

Deductibles are the amount you must pay out of pocket before your insurance kicks in. It varies from insurance plan to plan.. Please know how much your your deductible isand how much you have paid prior to coming to the office. It helps us know how to help you with any expenses you may have.
 

Co-Pays

These are recurring expenses you have each time you have a service, such as an office visit. Co-pays are set by the insurance company, and we are not allowed to waive them.

We ask that you come prepared to pay the co-pay amount, to save the additional cost of billing you, which must be passed on to you.
 

Cancellations

There are no cancellation fees for office or therapy visits cancelled at least 24 hours in advance. There is a $25 fee for missed appointments, as this time could have been used for another patient.
